Discharge Team Support Worker

This role is offered on a fixed term/secondment basis for 9 months. The hours are part-time, 22.5 hours per week. To be part of a service that will bridge the gap between inpatient care and community services by facilitating more timely and effective discharges from hospital by providing enhanced support for people to continue their recovery in the community. The support worker will work as part of the Integrated Discharge service and work with the hospital MDT to create interventions which will be varied because they are 'person centred', and specific to the needs of the individual. The main aim is for the support worker to enable a person to engage within their home environment, and wider social and cultural environments to support their meaningful occupations so increasing their well-being and independence whilst self-managing risk. This includes providing advice, supporting role and skill development within familiar activities of daily living, practicing different ways of doing things and signposting to relevant services. The post holder will work with the individual prior to and following their discharge from hospital, and contribute to the development of discharge support plans and support their delivery in a community setting in line with the principles of social inclusion and recovery.Main duties of the job o To support the integrated discharge service to facilitate timely and effective discharge from hospital o To support service users with severe / complex mental health needs with their discharge from hospital o The role requires a person to work autonomously, with support from the band 7 Occupational Therapy (OT) Lead and the Occupational Therapy Team team and to build networks within the local community to support service users to develop their skills in their home environment and maintain meaningful roles and routines. o To Identify suitable packages of support from voluntary sectors providers o To work in close collaboration with the multi-disciplinary team, before and after someone is discharged, to provide a positive experience for patients on discharge

This high-level overview shows we are in a healthy position, with higher scores than average for comparable organisations, alongside a great response rate, indicating good staff engagement. However, we also know we have plenty of room for improvement in many areas. To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.
