Full Job Description
The role is central to the co-ordination of both complex and non-complexdischarges. The post will be based within the Patient Flow Hub, and haveresponsibility for overseeing non-complex and complex patient discharges within PTHB Community Hospitals. In the course of their duties will beresponsibility for;
Facilitating and/or assist with effective, efficient and timely discharge ofpatients from hospital. Particular emphasis is placed upon quickturnaround of patients. This may include complex multi-disciplinaryassessment signposting, liaising directly with patients and their families/Care Transfer Coordinators/ MDT's assisting in case conferences forpatients with complex health and social care, palliative care orrehabilitation or housing needs. Giving families support and information.
Support the patient flow hub, as well as working closely with a range ofprofessionals and external agencies within the hospital and in communityservices.
Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries.

Giving families support and information. Support the patient flow hub, as well as working closely with a range of professionals and external agencies within the hospital and in community services. Work as part of the flow hub and work effectively with the wider multi-disciplinary team and third
sector groups to facilitate patient discharges in a timely manner across Powys Supporting Ward staff and multi-disciplinary teams in facilitating effective, safe, and timely patient discharge.
