This is a patient-facing role with the majority of your time spent in ward areas liaising with patients and their families & members of the ward team. This role will really provide a good introduction to expand your knowledge and understanding of how our hospitals operate.
  • You'll ensure discharge planning commences from day one of admission liaising with the multi-disciplinary teams and family to ensure patients spend the minimum amount of time required in hospital. Your effective communication skills throughout all of this will be key. You will be the main point of contact relating to the discharge of patients on your ward area, linking in with social care and therapy services, utilising our IT systems. You'll need to use your initiative of course, but you will also have a supportive team around you to help. Your strong organisational skills will also come into play as you manage your workload and ensure tasks are completed timely and any concerns escalated. Our services are 7 days a week - teams are deployed to work on flexible shifts at either of the acute sites depending on business need.
