Full Job Description
The main aim of the discharge facilitator role is to provide proactive and effective support in the discharge process; to be a point of contact on the assigned wards for patients, families, carers, patient flow and other professionals. The discharge facilitator needs to be able to work to the priorities of allocated areas/wards as directed by the band 6 Integrated Discharge Nurse. They are responsible for identifying no criteria to reside. supported discharging patients and reporting on each individual patient on the wards they are allocated for their next step in their discharge journey. They will be required to support with early intervention for discharge planning with information gathering including conversations with appropriate parties inclusive of the ward., Attending MDT at allocated ward specific times and directly reporting back to the band 6 Integrated Discharge Nurse for support and escalation as required and to assign priority of tasks Early supported patient conversation and recording on the appropriate documentation record to affix in the medical notes Early escalation of complex cases and conversations to appropriate MDT members Same day and future confirmed planned discharge facilitation of pathways 1, 2 and 3 (those requiring increased health/social care support). Certain non-clinical referrals inclusive of SPOC, Red Cross, Freewheelers etc and chasing actions eg, key safes, microenvironments/ downstairs loving, house clean etc. to reduce length of stay delays To support with the Transport Hub and risk assessment queries Out of area discharge support for non clinical support. Accurate data entry within all clinical systems Identifying and facilitating patient's early transfer to the discharge lounge (aiming pre-midday) and promoting further utilisation Support palliative care discharges as appropriate Discharge checklist completion Pathway 0 ward led support as work case load and capacity allows(eg, restart POC, calling RH/NH and organizing transport)
