The successful candidate will arrange the safe discharge out of hospital for our patients. Good communication will be needed to establish what services need to be set up to facilitate a smooth, safe transition out of hospital. Good working relationships will be needed with the multidisciplinary team to work together to determine what care and services are needed to safely discharge the patients. Good telephone skills will be needed and documenting in the patients records., Arranging discharges for ward patients. Discussion with patients and relatives to establish what support is needed to facilitate discharge out of hospital. Discussion with multidisciplinary team members including social services, the discharge management team and district nurses. Liaising with the discharge lounge to arrange discharge home. We are a 26 bed gastroenterology ward looking to recruit a Discharge Coordinator to our team. This post entails arranging safe discharges for our patients when being discharged from hospital. This will need good communication with patients, relatives and multidisciplinary team members within and outside of the hospital. This will include discussion with social workers and district nurses.,

  • Relevant NVQ Level 3/Level 3 Diploma in Health & Social Care or equivalent relevant experience.
  • Care Certificate or willingness to complete within first 6 months of commencement in post.
  • GCSE equivalent in English and Mathematics.
  • Desirable
  • Short courses relevant to undertake patient and clinical care duties.
  • Special Skills & Knowledge Essential
  • Good verbal and written command of
  • English.
  • Ability to act upon own initiative.
  • Good communication skills.
  • Able to work under some pressure.
  • Ability to show leadership
  • Ability to plan own work.
  • Ability to work calmly under pressure.
  • Willing to take and record patient
