Full Job Description
To provide a high standard of clinical leadership and supervision to the Digital Care Hub service delivery team, ensuring procedures, guidelines, and protocols are adopted by the service delivery team to ensure the provision of safe effective care to patients.
Support staff development to enhance service provision, and customer relationships.
As part of service delivery and improvement, you will participate in clinical discussions with stakeholders, such as ICB, PCNs, Community Teams, Out of Hours Services and care homes.
In pursuing these duties the post holder will ensure compliance with the NHS Constitution (details of which can be found at the following websites www.nhsemployers.org or www.dh.gov.uk/nhsconstitution), · Demonstrate professional accountability and adhere to the NMC or other (as appropriate) professional Code of Professional Conduct.
· Take responsibility for the assessment of patients who contact the digital care hub services via telephone and video link.
· Competently assess and triage patients using the digital care hub services, including the assessment, planning and evaluation of programmes of care.
· Participate in the multi-professional review of patients who have been referred to digital care hub services, including those recently discharged from the hospital.
· Develop competence of professional practice to advice and support patients, using appropriate protocols.
· Develop clinical skills in the triage and assessment of patients via the digital care hub, as well as within the hospital.
· Supervise and maintain accurate legal records using care planning, Airedale NHS Foundation Trust's and digital care hub's documentation.
· Offer help and advice to the patient's relatives and friends responding to their individual needs in a caring and supportive manner.
· Collaborate with ward and community staff to provide the appropriate delivery of patient care and clinical and professional advice.
· Act as a clinical role model, which reflects the values, behaviours and clinical standards of the Trust and digital care hub.
· Adhere to Airedale NHS Foundation Trust's policies and procedures relating to Health and Safety.
· Consult and co-operate with clinical and support staff from all areas.
Managerial Skills
· Responsible for overseeing eRostering ensuring adequate staffing levels across the team
· Approve annual leave in line with agreed staffing levels
· Ensure all PDRs are completed for all service delivery staff within the Digital Care Hub
· Responsible for managing sickness and implementing the Trust's sickness absence policy for the service delivery team
· Support the DCH Business Manager with recruitment
· Responsible for the induction of new staff
· Responsible for ensuring all staff attend their mandatory training
· Responsible for delivery of training for new services/existing service developments to all staff
· Support service delivery staff in professional development
· Act as a deputy along with the DCH Business Manager for the service delivery team in the absence of the Head of Digital Care Hub and Nurse Consultant
· Inform the Head of Digital Care Hub and Nurse Consultant regarding accidents, complaints, untoward incidents and other significant events in the Digital Care Hub.
· Ensure as far as reasonably practicable the health and safety of hospital personnel, patients, relatives and visitors whilst in the Trust and Digital Care Hub.
· Supervise and ensure the safe use and efficient management of equipment and supplies.
· Provide support to all team members especially in times of stress, learning to recognise this in self and others.
· Ensure staff members are supported in developing their confidence, and competence in managing their specialist areas.
· Ensure all service delivery staff are proactively developing their specialist area wherever time allows, to ensure on going service improvement.
· Provide clinical supervision as required.
· Ensure clinical guidelines, protocols, and procedures are available and up to date and that staff adhere to them.
· Maintain a high standard of clinical care provided by the hub to patients, and their carers.
An exciting opportunity has arisen for an enthusiastic individual to join the Digital Care Hub as a Clinical Team Leader. The post holder must be a practising non-medical prescriber and have an understanding of providing digitally enabled clinical services and their impact.
