Full Job Description

Manage a designated caseload of patients who may have divers and complex dietetic needs.
Develop individualised holistic care plans which addresses nutritional status, weight, recommended dietary modifications, lifestyle factors, behaviour change, education, physical activity and where appropriate condition management.
Support patient care through implementation of dietetic care pathways.
Act as source of expertise in nutrition for medical staff, nursing staff and other healthcare professional.
Liaise with healthcare professionals within and outside the Trust

Are you soon to qualify as a Dietitian or are you a Band 5 looking to gain more experience?
