Full Job Description

As a key member of the Adult Diabetes Service, you will provide technical support and education to people with diabetes (PWD) using all forms of diabetes technology.

The role is key to increasing digital access for all our patients and rolling out hybrid closed loop technology.

This post holder will support the Diabetes Specialist Nursing (DSN) to implement Hybrid Closed Loop insulin pump systems within the adult service with an aim to reducing health inequalities and improving health outcomes., 1. To support the DSNs with insulin pump training for PWD starting pumps, upgrading their system and initiating hybrid closed loop systems

2. To demonstrate and support the setup of glucose monitoring systems associated readers, smartphone apps and online platforms as needed.

3. To facilitate or supervise the insertion of sensors / sub cutaneous cannulas via sterile procedure.

4. To support the PWD to download and synchronise devices for sharing clinical information.

5. To support diabetes HCL clinics to ensure PWD attending have successfully shared clinical data prior to review

6. To provide support with problems with diabetes technology via telephone, email, video call or face to face.

7. Within competency, recognise abnormalities in data uploaded from insulin pumps and escalate to registered nurse for management.

8. To coordinate the HCL pathway, working closely with the Diabetes Nurse team administrator, including booking of group sessions and ordering of appropriate equipment via SBS system prior to HCL initiation sessions if needed.

9. Work closely with external insulin pump company representatives to facilitate education sessions.

10. To participate in clinical discussions/meetings regarding patients' care and to work collaboratively with a range of professionals in statutory and non-statutory organisations and agencies.

Experience of Diabetes Technology initiation including HCL systems is desirable for this role.
