Full Job Description
We are looking for a motivated individual to work full time and deliver a comprehensive, high quality evidenced based Diabetes Specialist Nursing service to people with diabetes.
The role covers in-patient, out-patient and ante-natal care, actively contributing to the planning and development of the Diabetes services. Providing an advanced level of practice that encompasses aspects of education, research and management., You will be responsible and accountable for an identified case load of people with diabetes, proactively promoting health enhancing activities and behaviours, working with the patient / carer and the MDT team to reduce unplanned hospital admissions and co-ordinate care across all settings of the care pathway. Promote and ensure through ongoing review and monitoring that people with diabetes are treated in line with current guidelines and that the principles of dignity and respect are achieved and embedded within the team.
