Full Job Description

As the nurse specialist within the defined sphere of practice the post holder is responsible for providing specialist knowledge care to patients. They will act as the first point of contact who have need to access the specialist service. They will undertake a range of duties to ensure the effective delivery of care including undertaking specialist procedures, providing specialist advice, structured education and ongoing support to their client group, based on individual need.

The nurse specialist will support the development and implementation of the integrated diabetes service across the Northern Lincolnshire and Goole Clinical Commissioning Groups (CCG) in collaboration with other community providers to improve the quality of diabetes care.

Main duties of the job

Participates in developing specialist programmes of care for a caseload of patient

Responsible for providing ongoing care to the caseload of patients

Uses specialist knowledge and expertise to assess, plan, implement and evaluate care interventions for the patient

Uses specialist knowledge and expertise to adjust insulin and oral hypoglycaemic agent doses within protocol guidelines

Uses expertise to recommend adjustments/amendments to treatment plans, in consultation with patients to members of the multi-professional team
