Full Job Description
We are looking for an experienced band 7 Dietitian to join our Diabetes and Endocrine team, to provide excellent inpatient and outpatient diabetes care. You will join a dynamic team based at Homerton Hospital .We are a responsive service meeting the needs of an expanding local population, the service provides a number of routine and specialist services, including; providing multidisciplinary clinic support, dietitian and nurse-led clinics, ante-natal and pre-conception clinic, insulin pump clinic, transition clinic, virtual clinic, emergency reviews and managing routine and urgent ward referrals. We are also a recognised DAFNE centre. Applicants should possess an in-depth clinical knowledge of diabetes, which should be supported by recent experience in caring for people with diabetes. In addition, some experience of in-patient work and nutrition support/ enteral feeding. Applicants should also have excellent communication and clinical skills, time management skills and a commitment to evidence. Main duties of the job
- To provide advanced dietetic assessment to inpatients with Diabetes, including those with diverse presentations and highly complex physical and psychological conditions, to develop and deliver an individualised treatment programme in line with the evidence-based guidelines. To liaise and work with acute inpatient dietitians when needed.
- To be a dietetic lead for inpatients with diabetes that may have complex nutrition needs in all areas of the Hospital. To provide first line diabetes nutrition advice prior to discharge to the community or Diabetes team at the Hackney Diabetes Centre.
- To provide a diabetes dietetic service to referred patients and their carers attending the Hackney Diabetes Centre or the ante-natal service at Homerton Hospital in an outpatient setting. This may include patients post discharge, new to insulin, patients on insulin pump therapy or gestational diabetes.
- To develop and provide a dietetic diabetes service to the population of City and Hackney as part of the multidisciplinary diabetes team, acting as a key dietetic resource within the team.
- To provide clinical supervision and guidance for junior members of staff within the acute dietetic team and diabetes team.
- To support patients with diabetes utilising the technologies such as CGM, insulin pump therapy to achieve positive clinical outcomes.

- BSc Nutrition & Dietetics or relevant postgraduate qualification
- HCPC registered as a Dietitian Desirable
- Nationally recognised course in CSII/ insulin pumps
- BIDAC or DAFNE trained
- X-PERT/ DESMOND trained
- Behaviour Change training, Excellent oral and written communication skills with people from a variety of backgrounds e.g. via advocacy/interpreters
- Experience of delivering undergraduate student supervision and clinical education to graduate standard in area of specialty
- Ability to work effectively single handedly with individuals and groups
- Ability to demonstrate behaviour change facilitation/ motivational interviewing
- Ability to problem solve, analyse complex clinical problems and create practical solutions to provide best clinical care Leadership skills
- Ability to communicate life threatening/complex medical conditions in a sensitive and easily understandable language

- Experience of working with patients with multiple pathologies, undertaking assessment, making treatment and discharge plans Desirable

- Demonstrate knowledge and understanding of managing nutritional in patients with physical and psychological impairment.
