If you are an enthusiastic, experienced Community Staff Nurse, this is an exciting opportunity to enhance and develop your leadership knowledge and skills within the dynamic role of a Community Nursing Deputy Team Leader., This permanent position will see the successful candidate working alongside the District Nurse Team Leader, assisting in managing and leading a Community Nursing Team. In addition, you will work in collaboration with the wider Integrated Community Care/Primary Care services to ensure the delivery of safe, effective and patient focused planned and unplanned care at home., In collaboration with the District Nurse Team Leader, lead and manage the caseload of the Community Nursing Team, monitoring standards of care, delegating responsibilities and supervise team members as appropriate.
Develop and maintain appropriate communication systems. Working closely with patients/carers and families and other members of the integrated care team and statutory and voluntary agencies.
Working in partnership with other agencies, act as care manager for identified patients in order to prevent inappropriate hospital admission.
In collaboration with the wider integrated care team provide care planning, support, treatment and monitoring to patients on the general case load.
In collaboration with the Team Leader, manage resources effectively and advise on skill mix requirements for the team.
In collaboration with the Team Leader, assist with the management of team budgets.
Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence based practice and participation in clinical audit.
In collaboration with the Team Leader assist with performance reviews within the team in order to contribute to team and corporate objectives, encouraging and supporting development of staff and maintenance of personal development plans.
Ensure team members undertake mandatory training requirements.
