Full Job Description
- Support the Clinical Lead in the operational management and day-to-day delivery of the EOLUCS team.
- Provide clinical leadership, specialist assessment and decision-making for patients receiving end of life care in the community.
- Work closely with internal teams and external health and social care partners to deliver coordinated, patient-centred care.
- Deputise for the Clinical Lead when required., This is a full-time post of 37.5 hours per week, with working pattern options including 5 days per week or compressed hours across 4 days. The role is primarily based atSt Luke's Hospice with community visits as required. - Registered Adult Nurse with current NMC registration.
- Experience of clinical leadership, team coordination and supporting staff development.
- Strong assessment, communication and partnership-working skills.
- A commitment to delivering high-quality, compassionate end of life care.
- Experience in palliative and end of life care, community care, or complex care coordination would be advantageous.
