Dementia Care Clinical Specialist

We are seeking a person who enjoys working with people living with dementia and who values the importance of person-centered care, for a fixed term contract for 10 months. Secondments may be available for internal candidates with the agreement of your manager. The Dementia Support Service is a well-respected, supportive and hardworking team of support workers, nurses and occupational therapists who are dedicated to optimising the care of in-patients with dementia and enabling best outcomes for them at the point of discharge. As a dementia specialist practitioner your focus will be on providing specialist assessment and care planning for patients who present with complex needs, along with interventions to enable engagement in care. You will promote person centered approaches, working collaboratively with patients, their families/carers and staff. This is an exciting and rewarding post which rarely becomes available. We look forward to applications from people who take pride in their work and are passionate about putting the needs of people living with Dementia at the centre of their care., On a daily basis you will see in-patients referred to the team across different specialties in the hospital and will be expected to prioritise your caseload as well as that of the Dementia Support Workers. You will undertake specialist dementia needs assessments and care planning to enable a unified, person-centered approach to optimize care, working alongside families/carers and staff.

This role requires a creative approach to problem solving, and the proactive provision of interventions and reasonable adjustments to maintain skills, engagement in care and best outcomes. You will be working independently as well as working alongside the dementia support team and many teams. A thorough induction and regular supervision will be provided.
