CYP - MH Acute Liaison Practitioner

Offering timely triages and assessments to support their wellbeing with a view to preventing admissions to the acute wards.,

Role Overview:As a Clinical Practitioner within the children's services urgent care umbrella, you will assess children and young people referred to the service, collaborating closely with internal teams and external agencies to ensure coordinated, holistic, and effective care pathways.

We currently have a 12-month fixed-term/secondment vacancy for a Band 6 Clinical Practitioner to join the CYP - MH Crisis Acute Liaison Team based at the Caludon Centre in Coventry (on same site as UHCW) We are seeking clinicians with experience of risk management and safety planning, who want to make a positive difference to the lives and outcomes of children, young people and their parents/carers. Experience of working with children and young people is desirable but not essential and we therefore welcome applications from those working in wider services who regularly support individuals in crisis and collaboratively work with them to ensure safe and effective care is supported and delivered.,
• Experience in crisis or acute care, supporting mental health needs and assessment. Children's experience is desirable.
• Strong assessment, risk management and communication skills
• Ability to work independently and as part of a multidisciplinary team
