CRHT Senior Nurse Practitioner

We are seeking an enthusiastic, experienced and compassionate mental health practitioner to join the Crisis Resolution and Home Treatment Team within Black Country Healthcare NHS Foundation Trust. The team provides timely, intensive, person-centred assessment and treatment for adults experiencing an acute mental health crisis, supporting people safely in their own homes wherever clinically appropriate and helping to prevent avoidable hospital admission or enable earlier discharge.

Working as part of a skilled multidisciplinary team, you will assess mental health presentation and dynamic risk, contribute to collaborative care and safety planning, deliver evidence-based interventions, and work closely with service users, carers and partner services. You will need sound clinical judgement, effective communication skills and the ability to respond calmly and flexibly in a fast-paced environment.

The post involves working across a 24-hour, seven-day service, including evenings, weekends and bank holidays, in accordance with the requirements of the role.

Main duties of the job

Complete comprehensive mental health, risk, safeguarding and physical health assessments in community settings.

Coordinate intensive home treatment, developing and reviewing person-centred care and safety plans with service users and carers.

Use sound clinical judgement to manage changing presentation and risk, including crisis triage and admission gatekeeping.

Deliver evidence-based interventions and support safe medicines management within professional scope and Trust policy.

Work collaboratively with inpatient, community, primary care and partner services to prevent admission and support timely discharge.

Provide clinical leadership and support to colleagues, maintain high-quality records, contribute to service improvement and participate in the 24-hour, seven-day rota.
