The Virtual Wards (AOT, CARe and HHCT) service is a community based service providing short term care to people across Oxfordshire and we are looking to recruit an Coordinator Support to join our team. We are seeking an individual who is proactive and committed to work as part of a team and on their own. The team has two bases - Unipart House Business Centre and the Horton General Hospital. The post holder will be based at our Horton office. The postholder will play a pivotal role in the referral pathway by supporting the Patient Coordinator Support by making sure referrals are picked up in a timely manner. The postholder will also be responsible for ensuring visits are planned in the most appropriate and efficient way. The service operates from 7am-10pm 7 days a week, therefore the postholder is required to work 1 weekend every 3 weeks., 1. To be a member of the Hub Team with the responsibility of supporting the Service Coordinator to triage referrals, plan screening visits, plan initial service assessments and support hospital flow in a timely manner to reduce inappropriate admissions and work from the prioritisation protocol. 2. To work independently following trust and local policies, protocols and procedures with direction from the coordinator and management team. 3. To support with managing referrals from all points, providing support to other Service Coordinators where required, potentially from a different base. 4. The create and maintain relationships with internal, external, out of county and system partners, and have direct liaison with the service senior management team (SMT) to provide feedback about Service capacity and flow status in the absence of the Service Coordinator. 5. To plan and allocate the staff rota efficiently, creating patient profiles on Pass system, allocation of support worker visits on Pass system, updating Live Screens, Rev Cycle and all other administrative duties as required. 6. To cover the Service Coordinator post in times of absence, including weekends. 7. To allocate patient reviews, to case managers. 8. To hand information over to Case Managers, Coordinators and Service Support Manager regarding changes in patient needs / packages of care., The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

  • Good knowledge of health and social care practices
  • Computer skills with a good understanding of IT and whole systems, Experience of working with an MDT, including Social Workers and associated professionals
  • Experience of clinical rotas and staffing
  • Experience of working across professional disciplines.
  • Desirable criteria
  • Experience working in a community setting, Ability to work autonomously with individuals in a variety of settings following trust & local policies, procedures & protocols
  • Ability to screen, assess and review for suitability for the Virtual Ward
  • To be able to critically examine and evaluate own clinical performance, initiate changes in work practice and integrate new knowledge into practice.
  • Demonstrate an understanding of social care provision and be able to communicate this to staff, service users and their representatives
  • Demonstrate problem solving approach.
  • To plan and allocate the rota effectively, taking into consideration the Virtual Ward budget
  • Understand and apply principles of risk management to clinical practice.
  • Understand scope/role of the Virtual Ward and team dynamics.
  • Ability to prioritise designated caseload.
  • Ability to collect and record accurate statistical information
  • Ability to proactively resource information to address patient's needs in a timely manner.
  • Effective written and verbal/nonverbal communication skills.
  • Ability to be actively involved, prepared and participate in system wide teleconferences, and provide feedback to a range of professionals/SMT.
  • Ability to appreciate the sensitivity, complexity of context and culture and diversity within clinical practice.
  • Ability to manage and recognise differences of opinion in both patient related and inter-professional settings coping positively with interpersonal tensions/barriers, and communicating effectively.
  • Understand values and principles of audit and research.
  • To undertake surveys/audits as necessary for own work and service development.
  • Flexible in approach and changing needs of the Directorate
  • Willingness to undergo further training as required
  • Desirable criteria
  • Able to draw upon and apply previous experience to identify and address problems with clients, and provide rationale for the chosen theoretical framework in which intervention is to be based.
  • Able to contribute to audit and quality assurance programmes, To be able to work flexibly across the working week between 7am - 10pm. including Bank Holidays, across a range of bases that the Virtual Ward runs from .
