Contact Cares Nurse Assessor

An exciting and challenging opportunity has arisen for a Registered Adult Nurse to work in our Contact Cares Front Door team for St Helens on a permanent basis. You will be a valued member of an increasing team working in close partnership with the Local Authority. There are a number of elements within Contact Cares and you will be diverse in your ability to offer your skills in each area. We are the first port of call 'Front Door' for services within St. Helens and you will be part of that service development. Service hours are 08:00-20:00 7 days per week therefore shift work will be a requirement of this role. As a highly committed nurse, looking for fresh challenges, you will have the unique opportunity to work as an autonomous practitioner within our team undertaking holistic assessments, providing relevant treatment and providing high standards of care to all who access our service. The Contact Cares Team provides a wide range of services to our local population, Acute Services, Out of Hours Services and Primary Care. An Enhanced DBS with barred lists for both Adults & Children is required for this post.Main duties of the job To provide a responsive, integrated service within the district of St. Helens, working in partnership with the Local Authority. Provision of high quality nursing assessments across a range of conditions. This will include referring on to partnership organisations, disciplines and liaising with all members of Multidisciplinary Teams.
