Oxleas NHS Foundation Trust was formed in April 1995 and is a provider of Community and Hospital Mental Health and Learning DisabilityServices for Greenwich, Bexley and Bromley. It is a specialist provider of Forensic and Challenging Behaviour services for Bexley, Greenwich,Lewisham, North Southwark, Bromley and Kent Prisons. Since 2010, Greenwich ALD - The post holder will be part of the Mental Health and Challenging Behaviour Team and will undertake clinical assessments ofpatients with mental health and contribute to care plans accordingly.

They will conduct a minimum of 4 Mental Health out-patient clinics per week. You will have a caseload of between 80 and 100 Mental Health(including Challenging Behaviour and Dementia). Diagnoses will include psychosis, anxiety, OCD, affective disorders, and dementia, autism,ADHD etc.

The ALD service - Provides specialist health care and services for people over the age of 18 with learning disabilities living in Greenwich,Bromley, and Bexley boroughs. The teams aim to provide high quality, evidence-based assessment, monitoring and intervention to support peoplewith learning disabilities to become and stay well, independent and to live a meaningful life. Service users are empowered to be active participantsin their own care and in service development., At Oxleas NHS Foundation Trust, we offer a wide range of NHS healthcare services to people living in South-East London and to people in prisonacross England. Our wide array of services includes community health care, such as district nursing and speech and language therapy, care forpeople with learning disabilities and mental health such as psychiatry, nursing and therapies.
