Full Job Description
The Worcester City Neighbourhood Mental Health Team (NMHT) underwent Community Mental Health Transformation in 2021, following the principles outlined in the NHS Community Mental Health Framework (2019). Our way of working aims to improve access, reduce fragmentation, and help people maintain wellbeing, stability, and meaningful lives within their own communities. The team provides accessible, person-centred mental health care supported by local Primary Care Networks. Its purpose is to offer holistic, trauma-informed support for adults with a range of mental health needs, including severe, complex and enduring conditions. The team aims to meet the needs of both people with Severe Mental Illness (SMI) as well as those with trauma histories, personality difficulties, co-morbid substance use, long-term physical health conditions, and social adversity. To this end, the team works collaboratively with primary care, social care, voluntary sector partners and community services to deliver integrated, recovery-focused care. The aim is seamless care across organisational boundaries with a no wrong front door referral policy and a step-up and step-down flow of patient care between primary and secondary services.
