Full Job Description
The successful candidate will contribute to delivering high-quality frailty care across acute and same-day emergency settings.,
- Provide consultant-delivered care within Frailty SDEC and front door frailty pathways
- Undertake Comprehensive Geriatric Assessment (CGA)
- Support early clinical decision-making and safe discharge planning
- Contribute to admission avoidance and same-day care pathways
- Work collaboratively with ED, acute medicine, therapy teams and community partners
- Participate in the Frailty Virtual Ward and wider frailty pathways
- Contribute to service development, audit, and quality improvement activity
- Support teaching and supervision of junior doctors and multidisciplinary staff Job Plan The post is based on 10 Programmed Activities (PAs):
- 8.5 Direct Clinical Care (DCC)
- 1.5 Supporting Professional Activities (SPA) Indicative activities include:
- Frailty SDEC sessions
- Front door frailty assessment (ED/CDU)
- Optional inpatient sessions depending on service need and interests
- SPA time for education, audit, governance and service development The job plan is flexible and will be agreed with the successful candidate. Those who wish to work part time will be welcomed.
