Full Job Description

We are introducing two key services co-located within Ward 22:

  • Frailty Same Day Emergency Care (Frailty SDEC) - A high-turnover, same-day assessment unit to reduce unnecessary admissions through rapid access to CGA and multidisciplinary care.
  • Acute Frailty Unit (AFU) - A short stay unit offering specialist-led treatment, early therapy input, and integrated discharge planning.
  • Your Role:
  • Lead delivery of Comprehensive Geriatric Assessment from the point of admission.
  • Provide senior clinical decision-making and mentor junior staff.
  • Collaborate across the Acute and Frailty Pathways to improve flow and outcomes.
  • Drive innovation, quality improvement, and integrated working with community services.

  • Main duties of the job Support and further develop the geriatric service Support and develop the interface frailty services for ED and relevant clinical areas Support, lead and develop integrated care working with CCG's and community partners Be an advocate for the care of older people within the organization. Lead, support and mentor junior medical staff within the department and ensure training requirements are met., 1. Improving representation at senior levels of staff with disabilities, from black, Asian, and ethnic minorities background, identify as LGBTQ+ and women, through improved recruitment and leadership development 2. Widening access (anchor institution) and employability 3. Improving the experience of staff with disability 4. Improving the EDI literacy and confidence of trust staff through training and development 5. Making equalities mainstream
