Full Job Description
We are seeking a dedicated and experienced Coordinator to join our Connecting Lives service. The Connecting Lives team are responsible for the monitoring of placements for adults with a wide range of needs including Learning Disabilities, Physical Disabilities, Mental Health needs, Dementia or other vulnerabilities. The Connecting Lives Coordinator's responsibilities are support and monitor adult placements delivered by self employed Carers and Support Workers. In this role, you will; Required to undertake a number of diverse specialist or technical activities within a service function using skills that would have be gained through formal qualifications and/or considerable relevant work experience. Roles at this level are expected to work unsupervised for much of the time, accessing and assessing information independently. The Connecting lives coordinator's will be required to recruit, prepare, assess, approve, support and monitor self employed Connecting Lives Carers and Support Workers. To find placements with Connecting Lives Carers and Support Workers for vulnerable adults referred to the Scheme. To support the development of the Connecting Lives service for a range of service users., 1. Hold own case/work load under the direction of a qualified professional in support of increased service delivery, applying knowledge and skills to a range of activities demonstrating understanding of the service area, relevant procedures and guidelines. 2. Provide support and advice to identified groups or individuals as detailed within an appropriate assessment framework and make recommendations as required. To include the recruitment and assessment of Connecting Lives carers. 3. Undertake direct work with service users using appropriate methodologies and communication to ensure standards are met. 4. Introduce or give information and advice to service users about other services available to them to address other underlying needs. 5. Communicate effectively with multi agency partners, colleagues and service users, both verbally and in writing through the appropriate use of case notes and other record keeping within information sharing protocols and record keeping policies.
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check. Valid driving licence and own transport is an essential condition for this role., We are looking for an individual who is passionate about working in adult social care and who possesses the following skills and experience,
- Vocational qualification or experience to level 3 or 4 relevant to the service user group or equivalent.
- Significant practical experience of working with service user groups giving rise to a variety of technical skills and sound understanding of the services available, work practices and processes relevant to the role.
- Knowledge and understanding of safeguarding protocols and the threshold levels for service users groups.
- Ability to manage own work without close supervision Desirable Criteria:
- Ability to make analytical reasoning and recommendations about particular areas of work.
