Complex Care Response Specialist

More about the role:You'll support the set-up, stabilisation and delivery of complex care packages, including:
Supporting safe and timely hospital dischargesDelivering high-quality, person-centred careSupporting clients with daily living activities and promoting independenceBuilding routines that improve confidence and wellbeingProviding reassurance and continuity for clients and their familiesMentoring and supporting new Support WorkersWorking alongside Care Managers and Clinical ProfessionalsEnsuring medication, equipment and care supplies are in placeIdentifying concerns and escalating appropriately

Travel is an essential part of this role:

This is a regional role so you will need to be happy and able to travel across your designated region
This may include:
Travelling to assignments across the countryWorking on both live-in and live-out care packagesStaying away from home when required

To support you, we'll provide:
Paid business mileageAccommodation for assignments outside reasonable travelling distanceAccommodation for live-out packages where overnight stays are required

Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance.This is a role where your experience, compassion and confidence genuinely change lives.What we can offer you:
