Community Wound Clinic Nurse

Working across clinics and supporting Community Nursing Teams, you will collaborate closely with GPs, Primary Care Networks, and multidisciplinary colleagues to deliver seamless, patient-centred care. You will manage a caseload, applying clinical judgement to complex situations while acting as a role model for junior staff and students. The role also offers opportunities to contribute to service development, quality improvement initiatives, and the enhancement of specialist wound care services. Main duties of the job As a Wound Care Community Nurse, you will play a key role in delivering specialist wound care to patients with complex and hard-to-heal wounds in community clinics and patients' homes. Working within the Wound Management Centre and alongside the wider Community Nursing Team, you will assess, plan, deliver, and evaluate high-quality, evidence-based care that supports healing, improves outcomes, and enhances patient experience. You will support the effective day-to-day running of wound clinics, promoting patient independence through education, self-management, and personalised care planning. Working closely with Primary Care Networks, GPs, and multidisciplinary colleagues, you will contribute to seamless, patient-centred community care. This role requires excellent communication, organisation, and clinical decision-making skills, together with a strong understanding of community nursing, safeguarding, risk management, and clinical governance. You will maintain accurate clinical records, support quality improvement and patient safety initiatives, and escalate concerns when appropriate. • You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
• You'll need to have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
• You will be a registered health care professional with the Nursing and Midwifery Council
• A full, valid UK driving licence or a full driving licence issued by an EU country
