Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Are you a passionate and experienced nurse looking to make a real difference in wound care? Join our specialist Wound Management Centre team, delivering high-quality care to patients with complex and hard-to-heal wounds across community clinics and patients' homes.

You will provide assessment, planning, delivery, and evaluation of wound care interventions, helping patients achieve the best possible outcomes while promoting independence and self-management. Working across clinics and supporting Community Nursing Teams, you will collaborate closely with GPs, Primary Care Networks, and multidisciplinary colleagues to deliver seamless, patient-centred care.

You will manage a caseload, applying clinical judgement to complex situations while acting as a role model for junior staff and students. The role also offers opportunities to contribute to service development, quality improvement initiatives, and the enhancement of specialist wound care services.

If you are motivated and committed to outstanding community-based care, we would love to hear from you., As a Wound Care Community Nurse, you will play a key role in delivering specialist wound care to patients with complex and hard-to-heal wounds in community clinics and patients' homes. Working within the Wound Management Centre and alongside the wider Community Nursing Team, you will assess, plan, deliver, and evaluate high-quality, evidence-based care that supports healing, improves outcomes, and enhances patient experience.

You will support the effective day-to-day running of wound clinics, promoting patient independence through education, self-management, and personalised care planning. Working closely with Primary Care Networks, GPs, and multidisciplinary colleagues, you will contribute to seamless, patient-centred community care.

This role requires excellent communication, organisation, and clinical decision-making skills, together with a strong understanding of community nursing, safeguarding, risk management, and clinical governance. You will maintain accurate clinical records, support quality improvement and patient safety initiatives, and escalate concerns when appropriate.
