Community Support Worker - Transport

The post holder will provide support to patients to ensure safe and timely discharges from hospital to their community discharge destination. Clear communication between the Hospital Discharge Team, Discharge to Assess Hub, D2A, ICAH and the wards to ensure safe timely discharge of all patients . Safe, compassionate and person centred care to be given to support patients.,

• To support the Discharge Team in facilitating timely discharges for complex patients
• Provide safe, compassionate and person centred care and support to patients during their transfer from hospital to their community discharge destination
• Home visits and lone working without direct supervision
• Transfer patients to the discharge lounge if required prior to supporting on community transport
• Workload allocated by a registered professional
• Flexible working pattern to meet the needs of the service
• Excellent communication skills with patients, relatives and colleagues
• Have an understanding of services available within the community
• Maintain excellent written patient records, demonstrating patient confidentiality in accordance with Caldicott guidelines
• Maintain close liaison with colleagues and to communicate appropriately with other departments / services as required. Ensuring effective feedback regarding patient conditions to appropriate health care professionals
