Full Job Description
Are you passionate about helping people remain safe and independent in their own homes? We are looking for a motivated and compassionateCommunity Support Worker to join our falls response service, within the Urgent Community Response Team. The successful candidate will respond to adults who have experienced a fall within the community, providing timely assessment, support, and intervention to help avoid unnecessary hospital admissions and promote recovery at home. When you're not out supporting our patients, you will be in the Single Point of Access HUB as part of the call handling team, you will be answering the phones, taking referrals from member of the public and healthcare professionals and supporting the wider teams. The Community Referral Hub will soon be operating 24/7, 7 days a week, 365 days a year. Due to the role requirements, you ideally need to be able to work 12 hour shifts and be able to work 1 weekend a month. This role will involve working rotational day and night shifts. Flexibility is a vital part of this role to support service needs such as annual leave, sickness or service pressure. Flexibility goes both way, we offer a range of flexibility to support you. This can be discussed at interview., Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.
- Assess the patients home to ensure that they are not injured and it is safe for you to assist
- Support the patient in regaining an upright position, with the support of a multitude of equipment
- Support the patient with any basic care requirements
- Complete manual handling as appropriate for the scenario and in line with manual handling guidelines
- Complete the IStumble evaluation form using our EMIS system for the patient
- Complete the patient referral in its entirety
- Complete any onwards referrals for the patient
- Add any follow up calls & complete when necessary
- Be part of the whole CRH Team
- Take phone calls from community services, patients and other HCP's and ASC professionals and complete the referrals on the EMIS system
Being able to multi-task and using an array of different systems - Using a computer will be a big part of your role
- Being able to either touch type or type confidently at a steady speed
- Understanding that you are part of a much wider team
- Being adaptive and moving quickly with new processes that are implemented
- Having a positive and a can do attitude, especially when face to face with the patients.
- Friendly and confident telephone manner
