Full Job Description

As a Community Support Worker (CSW) you will be working within the Intensive Community Care pathway and Urgent Community Response team covering Oxfordshire county. Working independently and under direction and support of a registered clinical practitioner, the CSW will visit people at home who are experiencing an urgent health and social care crisis. The CSW helping the person in their immediate recovery and enabling them to remain at home or place of residence (where safe to do so) without the need for an unnecessary hospital conveyance. The CTS role is ideally suited for someone who is compassionate, resilient and motivated to make a real difference. Community urgent same day care services operate county wide and the role will be working shift patterns and occasional weekends between 8am and 8pm, 365 days a year. The post-holder may be required to work flexibly across shift patterns and will operate within a locality area, however the services are county wide and some travel will be required. Although this vacancy is advertised for 25 hours, we may be able to offer the role as a full time position.,

  • Deliver care to people within their own homes by supporting registered and unregistered colleagues in the delivery of safe and effective patient/client care and management plans and other duties under guidance and supervision.
  • Work collaboratively with colleagues, carers and partner agencies to promote continuity of care and effective communication across services and organisations.
  • To establish and maintain professional working relationship[s within urgent community response. community response, South Central Ambulance Service (SCAS). Primary Care and with patients/clients, carers, and relatives.
  • Accepting and relaying information and messages as required and reporting any changes in condition to a registered practitioner. This may include staff from other agencies or localities when a patient/client's care is being transferred.
