Are you an experienced Community Support Worker with a strong understanding of joint health funding, support planning, reviews and recharges? About the Role We are seeking a motivated and skilled individual to join our team on a 12-month secondment to support the review, monitoring and improvement of joint-funded care packages across Adult Social Care and Health. The role offers a unique opportunity to develop specialist expertise whilst working closely with Care Pathway teams, Health colleagues, Finance and Commissioning partners.
- Has experience working within Adult Social Care and applying Care Act principles in practice.
- Has knowledge or experience of joint health funding, Continuing Healthcare, recharge processes, support planning and commissioning arrangements.
- Can confidently review care packages, identify funding opportunities and resolve complex funding queries.
- Has experience of working collaboratively with Health partners and other agencies in a multi-disciplinary environment.

- Hold a Level 2 in Maths and English and a relevant Health & Social Care Qualification.
- Have relevant experience of working with adults in a social care setting and of working with other agencies within a fieldwork environment.
- Understand the need to undertake need and risk assessments and the issues/needs that adults and their families/carers may have.
- Have a good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector.
- Be a confident communicator who is skilled in negotiation, persuasion, assertiveness, and conflict management, and able to build effective working relationships.
- Be able to recognise the limits of own accountability and responsibility, and to seek appropriate support and advice when required.
- Be able to manage a caseload and work to clear objectives. In addition, we also expect you to share and to delivering accessible and inclusive services which meet the needs of all of our residents.
