Community Support Worker

The post holder will provide person-centred support to older adults with mental health needs, dementia, cognitive impairment, and complex social circumstances. Key responsibilities include developing and maintaining therapeutic relationships, promoting independence, recovery and wellbeing, supporting individuals to achieve care plan outcomes, and encouraging participation in meaningful activities and community engagement. The role requires monitoring and reporting changes in mental state, behaviour, physical health, and risk, escalating concerns appropriately to qualified practitioners. The post holder will recognise and respond to safeguarding concerns, contribute to risk management plans, and work in accordance with lone-working, health and safety, and safeguarding procedures. Support the assessment, care planning, and delivery of care for older adults experiencing functional mental health difficulties, cognitive impairment, or dementia. Working under the supervision of qualified practitioners, they will provide evidence-based interventions that promote recovery, independence, wellbeing, and carer support. The post holder will identify and escalate risks, safeguarding concerns, deterioration in mental health, carer breakdown, or other barriers to recovery. The role requires effective communication with service users, families, carers, and professionals, providing information about care plans, treatment options, and available community resources. The post holder will work collaboratively within a multidisciplinary team, contributing to MDT meetings, joint assessments, care reviews, and discharge planning to ensure coordinated, person-centred care. The post holder will maintain accurate clinical records, comply with safeguarding, information governance, clinical governance, and risk management procedures, and use digital systems effectively for communication and documentation. Working across community, clinic, and remote settings, the post holder must safely undertake home visits, manage a varied workload, prioritise competing demands, maintain professional boundaries, and work independently when required., The Support Worker will assist in implementing care and treatment plans, contribute observations to assessments and reviews, and maintain accurate clinical records using electronic patient record systems. They will work collaboratively with service users, families, and carers, providing information, practical support, and signposting to relevant services where appropriate.

The post holder will work as part of a multidisciplinary team alongside Community Psychiatric Nurses, Occupational Therapists, Psychiatrists, Social Workers, Dementia Specialists, and Primary Care Services. Effective verbal and written communication skills are essential, particularly when supporting individuals with dementia, sensory impairments, communication difficulties, or mental health conditions. Working within community settings, the post holder must be able to undertake home visits, travel across the locality, manage their workload effectively, and use IT systems competently. Knowledge of older adult mental health conditions, safeguarding, the Mental Capacity Act, and person-centred, recovery-focused practice is required. The role demands compassion, resilience, professionalism, organisational skills, and the ability to work both independently and as part of a team.
