Community Support Worker

We're excited to be recruiting to our newly created Access Team - the "front door" into Adult Social Care. This is a unique opportunity to be part of a dynamic service that provides fast, short-term support to people when they need it most.What will you be doing? As a Community Support Worker in the Access Team, you'll play a vital role in delivering timely, strengths-based interventions to adults with a range of needs, including mental health, physical disabilities, learning difficulties, and complex social situations. Working in a fast-paced, responsive environment, you will:

• Provide early intervention and short-term support to prevent escalation of needs
• Act as a key contact at the "front door" of Adult Social Care, helping people access the right support quickly
• Promote independence, wellbeing, and community-based solutions
• Help reduce the risk of hospital admission or long-term care placements
• Work closely with individuals experiencing challenges such as budgeting issues, hoarding, supporting with debt-management, organising paperwork and maintaining safe living environments
• Collaborate with partner agencies and community services to build strong support networks

To succeed in this role, you will bring:
• Experience of working in community settings, supporting adults with a range of needs
• A good understanding of real-life challenges such as budgeting, hoarding, and maintaining independence
• A proactive, flexible approach and the ability to work effectively in a busy, fast-moving environment
• Strong communication skills, with the ability to build rapport and work collaboratively with individuals and partner agencies
• The ability to write clear, concise, and accurate records
You'll also have:
• GCSEs (or equivalent)
• NVQ/QCF Level 2 in Social Care (or equivalent)
• Proven experience in health, social care, or a related field
