An exciting opportunity has arisen to join the Mental Health Services for Older People Community Team in Whitby.

The team support individuals and families within the community so you will work alongside a professional and caring multi-disciplinary team to provide high quality care to those accessing the service. Within this role you will develop a good knowledge of local services within the area and support individuals in the community and local care/residential homes. You will need to be able to communicate well with a wide range of individuals, adapting your communication style according to individual need in a responsiveand caring manner.

We are looking for people who can work relatively independently, however you will be supported regularly with supervision and support and professional development.

You will need to travel independently over a large geographical area and will require suitable transport., As a Community Mental Health Support Worker, you will work with older adults, families and their carers to provide ongoing support, therapeutic interventions and practical assistance under the supervision of a Registered Nurse.

The role will involve:

Providing a safe, patient centred and effective clinical activities in the community setting.

Monitoring of mental and physical health, under the direction and supervision of registered staff

Delivery of non-pharmacological interventions on an individual and group basis, focussed on assisting the person to identify and achieve agreed goals

Providing an essential role in supporting patients within Care Homes to monitor and review mental health needs, and also to foster positive working relationships and effective joint working between services.

Provide timely physical interventions ensuring safe monitoring during medication changes and early detection of health concerns.

Carrying out ECG or venepuncture as required - training will be offered
