Full Job Description

We are seeking an enthusiastic and motivated Nurse to join our Community Stroke Therapy Team. In this rewarding role, you will utilise your specialist stroke knowledge and clinical expertise to deliver high-quality, patient-centred care across the New Forest. Responsibilities will include undertaking comprehensive holistic assessments, supporting the delivery of rehabilitation programmes, and completing 6-week and 6-month post-stroke reviews. Working collaboratively within a multidisciplinary team, you will play a key role in providing an efficient, responsive, and evidence-based service that supports stroke survivors through their recovery journey.

The stroke therapy service uniquely covers 3 areas, including Early Supported Stroke Discharge (ESD), a 16-bed inpatient unit (8 stroke and 8 medical rehabilitation beds).

You will participate in our IST programme, supervision structure, service development and our weekend rota supporting our 7 day a week ESD service., To provide specialist Stroke knowledge and skills to the Community Stroke Team.

To assess and treat own caseload, which may include complex presentations.

To work with patients to support them to achieve their personalised goals, optimum level of independence and quality of life. Working with the individual patient and in groups.

To maintain accurate and up to date patient records as an autonomous practitioner and in accordance with legal requirements, Trust standards and Professional Standards.

To provide supervision and education of junior colleagues, assistants and students.

To support Band 7s in daily management of the team and provision of therapy service, deputising in their absence.

To contribute to development and maintenance of an efficient and effective Stroke rehabilitation service.

To share the responsibility of covering a 7 day rota for CST, and changing duties at the last minute in order to support staff leave and sickness.

Ideally applicants will have a minimum of 2 years stroke/neuro experience, but those with less experience will be considered.
