Band 6 Occupational Therapist: Community Stroke Rehabilitation Team - Band5 to 6 development would be considered for the right candidate

30 hours

We are looking for a suitably experienced Occupational therapist to join our friendly and driven community stroke team.

As a valued member of our multidisciplinary team based at Infinity house Crewe, you will provide post stroke rehab to patients discharged home across the footprint of CCICP. You will be supported in this role by OT colleagues as well as the wider team to deliver quality, safe patient care. We have strong working relationship with our internal and external stakeholders and prioritise service improvements adopting a Continuous Improvement approach.

Main duties of the job

o To work autonomously to manage a defined complex patient caseload whilst working as part of a multidisciplinary team.

o To contribute to the delivery of the service in a variety of defined clinical settings, which may include rehabilitation settings, community clinics and the patient's home.

o To undertake specialist clinical assessment, taking into account all relevant information and clinical reasoning to formulate evidence based individualised treatment plans in agreement with the patient.
