An exciting opportunity has become available in South West Yorkshire Teaching Partnership NHS Foundation Trust, for a Community Staff Nurse in the Wakefield Tuberculosis Team.

This is a fantastic opportunity for an enthusiastic, self-motivated nurse to work under the guidance of a Specialist TB Nurse, working with patients across the community and their homes.

We are looking for a team player, who can work on their own initiative and provide a person-centred, holistic approach to patients independence and well-being.

The goals of the service are to identify and reduce the risk of people being newly diagnosed with Tuberculosis and provide high quality treatment and care for all people with TB

To support the senior nurses in carrying out health assessments and prescribed and supportive care.

The post will be based in Wakefield with close links to all relevant hospitals, community agencies and the health security agency., The successful applicant will be part of a proactive team assisting with the orientation of people new to the country referred for new entrant screening clinics and helping people navigate mainstream health services, There is a requirement to visit adults, children and families to support with active or latent TB at home across Wakefield during their treatment, including those with multi or extensively drug resistance TB

Deliver TB screening clinics, using phlebotomy skills for QuantiFERON testing in accordance with the Trust Clinical Standards and Guidelines and the NMC Code of Conduct/Guidelines

Record all patient contacts and activities accurately and contemporaneously using SystmOne

Participate in health promotion activities on all aspects of TB across health and social care, third sector organisations and any other organisation as necessary

Support and facilitate the direct or video observation therapy of patients undergoing prescribed treatment for TB, in line with TB management guidelines

Travel in a timely manner across Wakefield to undertake home visits and deliver clinics

Participate in large health screening programmes and disease management of patients under the direction of the senior nurses

Carry out BCG Vaccinations in clinic where appropriate

Work as a member of the Team and where appropriate liaise with regional TB teams to support and obtain clinical supervisions

Deputise for the Specialist Nurse in MDT clinics and meetings as required
