Community Staff Nurse (Physical and Mental Health)

Are you a registered nurse with a passion for improving outcomes for people with severe mental illness? We have an exciting opportunity to join our expanding Physical Health and Wellbeing Service, supporting the delivery of specialist clozapine care and physical health monitoring across Bradford District Care NHS Foundation Trust. This innovative service sits at the interface of mental and physical healthcare and plays a vital role in improving access to clozapine, supporting community-based treatment, reducing health inequalities and delivering care closer to home. The service supports people with severe mental illness through physical health assessment, specialist medicines monitoring, health promotion and integrated multidisciplinary care. We welcome applications from nurses registered in either Mental Health Nursing or Adult Nursing and would particularly welcome applications from individuals who are registered in two fields of nursing practice with the NMC. This is a unique opportunity to develop specialist skills in clozapine care, medicines optimisation, physical health and long-term condition management within mental health services., The post holder will be responsible for:

• Provide nursing assessment of patients/clients.
• Plan, develop and/or implement and evaluate programmes of care in a range of settings.
• Provide clinical supervision to other staff, students.
• May be responsible for work allocation and checking of tasks to support staff and/or less experienced staff.
• Provide health promotion information, advice.
