Community Staff Nurse (Multiple Locations)

As a Band 5 Community Nurse you will work to keep people safe & well at home. You will work autonomously but closely with a wider team and use your clinical judgment to support evidence-based care planning for people living home, in care homes or other settings while making sure those who use our services have a positive experience. You will be able to develop skills, maintain & share your previous experience.

• Holistic patient assessment, formulation & reviewing care plans
• Management of pressure ulcers & wounds
• Palliative & end of life care
• Bladder & bowel care including catheterisation
• Medicines Management
• Injections
• Phlebotomy
• Provision of equipment to support care

• Line management & supervision of junior staff
