An exciting opportunity has arisen for enthusiastic, compassionate, and motivated Registered Nurses to join our Community Nursing Team in Driffield on a temporary 1yr contract. As a Band 5 Community Nurse, you will work within patients' own homes, residential settings, and community environments, delivering high-quality nursing care that supports individuals to remain independent and receive treatment in the place they call home.This role offers the opportunity to develop a wide range of clinical skills while working as part of a supportive and experienced multidisciplinary team., What you'll do:
- Assess patients and develop, deliver and review individual care plans
- Provide safe nursing care using evidence-based practice and professional judgement
- Work closely with patients, families, carers and partner services
- Maintain clear, accurate patient records and report clinical concerns
- Support junior colleagues and contribute to service improvements
A Registered Nurse qualification and appropriate professional registration - Experience of working in a healthcare setting and as part of a team
- Good assessment, communication and basic digital skills
