Our work in Core Neighbourhoods is at the very heart of Stockport Neighbourhood Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers., The post holders will work with the Community Nurses and other multidisciplinary teams with the support of Mastercall GPs, to provide a first class service to the residents of Stockport.

We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers. The post holder should have good organisational and assessment skills be flexible and willing to cover the service between 08:00 - 18:00 over a 7 day period. Due to the nature of the role, it is essential that applicants are able to travel independently and efficiently across multiple community sites to meet the operational requirements of the role,

  • Mental effort to exert frequent concentration where work pattern is unpredictable. Ability to travel and be based at different locations. The post holder will be exposed to frequent distressing and occasionally highly distressing and emotional circumstances. ., Evidence of teaching and assessing students and junior members of staff.
  • Developing areas of expertise in community related practice. Tissue Viability/ Frailty/ end of life care
  • Experience of audit involvement in project work., Demonstrate knowledge of current local and national health agenda and developments.
  • Awareness of current clinical issues in out of hospital care, Demonstrate an understanding of an implication of an issue which could impact on community service provision., Current 1st level registration with NMC
  • Grade C / 4 (or equivalent) in GCSE English, Diploma in Nursing Practice or degree
  • Teaching and Assessing Certificate
  • Evidence of Health-related study at level 6/7, Diploma in Nursing Practice or degree
