We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

Main duties of the job

If you are looking for a rewarding career that brings new challenges every day, offers opportunities to develop your clinical skills, and enables you to make a real difference to patients in their own homes, then Community Nursing could be the role for you.

We offer excellent development opportunities and training directed towards your needs/goals.

As a Community Staff Nurse, you will assess, plan, deliver and evaluate nursing care for patients with a wide range of health needs in the community. You will work as part of a multidisciplinary team to promote independence, prevent unnecessary hospital admissions, and support patients to receive high-quality care in the place they call home.

We offer excellent development opportunities, including structured training, clinical supervision, and support tailored to your individual career goals.

Experience in adult community services is desirable but not essential. We welcome applications from newly qualified nurses and experienced nurses who are enthusiastic, motivated, hard working and passionate about delivering outstanding care in the community.

Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive - so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.
Our Community Nursing teams play a vital role in supporting patients to remain independent and receive care in their own homes. We pride ourselves on providing a supportive and inclusive working environment where staff are valued, respected, and encouraged to reach their full potential.
