This role offers the opportunity to work closely with general practice, therapy services and home visiting colleagues. Assess, plan, implement and evaluate individualised patient care, promoting self-care wherever possible while maintaining privacy, dignity and clinical effectiveness. Provide guidance and support to junior staff in delivering high-quality care, while actively assisting the District Nurse/ Senior Community Nurses in managing the healthcare environment.
