Community Staff Nurse

An exciting opportunity has arisen for the recruitment of a Community Staff Nurse based at Great Eccleston Nursing Team. The advertised role is a Temporary Fixed Term 12 month post (30.0 hours per week)., The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.

To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.

The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.

To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.

It is essential that you have a full UK driving licence and access to a car for this Community Nursing role.
