Community Staff Nurse

Are you a motivated and compassionate Registered Nurse looking to develop your career in community nursing? We are seeking an enthusiastic Band 5 Staff Nurse to join our dynamic Warrington District Nursing Team. In this role, you will deliver high-quality, person-centred care to patients in their own homes and community settings. Working under the supervision of the District Nurse, you will manage a delegated caseload, undertake comprehensive assessments, plan and evaluate care, and respond to changing health needs. You will build skills in key areas such as wound care, palliative and end-of-life care, chronic disease management, and catheter care, while maintaining accurate records in line with NMC standards. Collaboration is central to the role, and you will work closely with the multidisciplinary team, social care, and voluntary organisations to ensure coordinated, holistic care. Strong communication skills are essential when supporting patients, families, and carers, often in sensitive or complex situations. You will also contribute to the development of junior staff and students through mentoring and teaching. Perform comprehensive assessment of patient nursing needs as the named nurse. Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse. To support the team leader in the co-ordination of complex and continuing care packages. Maintain accurate records in accordance with NMC. Attend a multi-disciplinary case conference as the named nurse if appropriate. To communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care. To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations. To competently perform clinical skills related to the post. To be professionally responsible for a delegated caseload under the supervision of the District Nurse. To assess health needs at individual, practice and community level. To undertake chronic disease management reviews using appropriate template.
