Community Staff Nurse

The service is open 7 days a week covering and shifts will be between the hours of 8am and 6.30pm.

This post is part time 22.5 hours per week., Our Chester Central Community Care Team is based in The Fountains Building, Chester covering the surrounding geographic area . It's important that you have access to transport for work purposes, but of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
