Would you like to have autonomy in your role but still have the support of a multidisciplinary team? Would you like to see a variety of patients each day so that no day is the same? We are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking a Registered Nurse to join us in providing care and treatment to adults living in the area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards. To support you within your role, the Trust have an excellent learning and development department, access to further develop your clinical skills and leadership development are available and encouraged. Our service runs 7 days a week and therefore this position requires flexible working, and all requests will be considered. Access to a vehicle and possession of a full driving license is essential.Main duties of the job

  • Participate in the holistic assessment, implementation and evaluation of programmes of care and therapy for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs. Other settings include, residential homes, nursing homes, clinic's community centres and community hospitals.
  • Recognise and respond to the need for appropriate referral to other members of the Community Care Team or external organisations including appropriate escalation of potential Safeguarding risks.
  • Provide safe medicines management in your area, according to professional and organisational guidelines to ensure safety and efficacy.
  • Inform the Senior Community Nurse / Community Matron of any concerns with patients, relatives, visitors, or staff that may compromise patient care.
  • Develop and maintain constructive working relationships and liaise effectively with all members of the multidisciplinary team so that patients' needs are met.
  • Communicate effectively with colleagues, patients and carers so that information is shared in order to meet patients' needs.
  • Accountable for making autonomous clinical decisions in planning and prioritising patient care for the Team and across the Locality when required.
