Community Staff Nurse

A vacancy has become available within the community nursing service in Halton. Qualified nurses with enthusiasm and motivation are invited to apply. The role involves providing high-quality care to housebound patients, including palliative care, wound management, catheter maintenance, and phlebotomy. Candidates should possess the necessary skills to deliver patient care in home settings, demonstrate a commitment to working collaboratively within an integrated team, and be prepared to continuously develop their professional knowledge. Community nursing is advancing rapidly, and adaptability to change is essential. Flexibility, autonomous practice, and effective communication are key in ensuring safe, responsive community nursing services., The post holder plays a vital role in supporting the District Nursing team through the management of an assigned caseload. Key responsibilities encompass the assessment, planning, and evaluation of patient care needs. The incumbent works closely with the District Nurse to facilitate effective communication regarding patient care, maintain up-to-date care plans, and ensure seamless continuity of services, especially during reviews of complex care arrangements. Additional duties include contributing to mentoring, educational, and assessment activities. Collaboration with healthcare professionals and external agencies is integral to providing efficient and high-quality service delivery.
