Full Job Description

The Community Response Team is a team of skilled clinicians and clinical support workers working together to support patients being discharged from hospital following illness and preventing admissions into hospital for patients in their own home., To provide operational and administrative support to the Professional Lead in a way that is responsive to their needs and contributes to the effectiveness of their roles.

The post-holder will work within the Community Response Team (CRT) responding to a variety of situations on a daily basis.

We are looking for an enthusiastic, positive, organised person to join this team on a part time basis to support the Community Response Team lead to help manage the service. The successful candidate will be involved in a number of tasks including diary/capacity/pathway management, recruitment, meetings, training and co-ordination therefore will need to have a range of IT system skills, be able to manage the complexity of the role, highly motivated, work on your own initiative and have excellent communication/organisation skills.
