Full Job Description

The rehabilitation support worker assists physiotherapists/occupational therapists/nursing colleagues in providing treatment to patients. You will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximize patient's mobility and independence in daily activities in their own home. Main duties of the job

  • To establish a therapeutic relationship and communicate effectively with clients and carers to implement treatment plan with sufficient explanation to ensure informed consent.
  • To carry out treatment programs prescribed by the physiotherapist/occupational therapist, with designated clients.
  • To assist the Physiotherapists/Occupational Therapists with treatment sessions which may include walking practice, monitoring exercise programmes, equipment provision and teaching techniques of its use. Also including implementing rehabilitation programs to practice daily activities such as cooking, dressing and washing, possibly whilst using equipment or compensatory techniques.
  • To be aware of risk involved in providing care to clients and to ensure self and client safety.
  • To demonstrate good manual handling skills e.g. positioning and transferring clients, use of hoist, walking aids with moderate physical effort throughout the day.
  • To maintain accurate and up-to-date documentation which is consistent with legal, professional and organisational requirements.
  • On a daily basis, to be required to use basic computer skills e.g. e-mail, Internet, Intranet, Physio-tool, and use EMIS for report writing.
  • To assist in audit and/or research projects for service and professional development as required, Supporting Rehabilitation programs devised by Qualified Physiotherapists or Occupational Therapists
  • Reporting back to the case holder of any concerns noted/rehabilitation progress/review of client goals You will support the observation taking and identify any concerns or change in patterns, supporting the prevention of admission into hospital. You will attend weekly Multi disciplinary meetings to provide this feedback and support the progression of the clients.   • oExperience of client centred working, within a multidisciplinary team
  • oExperience of working with older people in a community rehabilitation setting
