Full Job Description
The post will involve working with healthcare assistants and clinicians to provide nursing interventions to patients in their home environment, maximising their independence and providing care to manage long term conditions as well as acute situations., This is a 12-month fixed-term opportunity for a Band 5 Community Nurse.
You will be able to demonstrate real enthusiasm for community nursing, alongside a commitment to ongoing professional development.
The team provides nursing care to housebound patients aged 18 and over within their own homes or chosen place of residence. You will work autonomously, supported by the Community Nursing Sister and Team Lead, delivering a range of nursing interventions and patient-centred care.
The service operates 7 days a week from 8am to 8pm, with flexible working available on request to support work-life balance.
A car owner/driver with business-use insurance cover is essential for this role.
You must be innovative in your approach and willing to work with other members of the integrated team to ensure a high quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training to extend your community nursing skills.

Applicants who do not have current valid UK NMC registration will not be considered for this post. Successful candidates must have post-preceptorship experience.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
